Please follow the instructions in the following form below for how students will use the account created for them and how you can create a parent account - and link the two together.  Students DO NOT create an account since one has been created for them by us.

23-24 PIAA Antietam Online Physical Form Instructions

The form below is what you will need to bring with you when you obtain your physical from your doctor.  You may need to fill out Section 5 for a doctor that is not your Primary Care Physician, but you will still need to input Section 5 information into your Students Big Teams Account.
