PIAA Physical Information/Instruction Forms

We will no longer accept the paper copy of the PIAA Physical Forms starting the 23-24 school year.  We use Arbiter to collect all information at the following website:

The form below is what you will need to bring with you when you obtain your physical from your doctor.  You may need to fill out Section 5 for a doctor that is not your Primary Care Physician, but you will still need to input Section 5 information into your Students Arbiter Account.

Hard copies of this Physical Form can be found in the AHS Main Office if you do not have access to a printer.
